Consultant pharmacists for assisted living
A Walgreens consultant pharmacist can conduct medical reviews, help seniors avoid medication-related complications and answer questions from patients taking medications prescribed by multiple doctors.
For residents
- Helps ensure a resident's medication therapy is appropriate, effective, safe and used correctly
- Helps identify, resolve and prevent medication-related problems including drug and drug-food interactions, dosing or therapeutic duplications interfering with therapy
- Provides medication recommendations related to specific diseases or conditions affecting seniors
- Identifies medications that may be unnecessary and/or cause adverse side effects
- Helps ensure adherence to the Beers' Criteria for safe use of medications specific to seniors
- Performs risk assessments to identify seniors at risk for behavioral disturbances, falls, diabetes and cognitive impairment
- Communicates with prescribers to suggest safer and, perhaps, less expensive therapies and medications
- Advocates wellness through health screenings and immunizations
For your community
- Observes adherence to medication pass and medication ordering procedures
- Assists with regulatory compliance related to surveyor's issues
- Provides your community with medication information specific to seniors
- Reviews and provides recommendations for your community's medication room, medication storage and medication destruction
- Provides educational programs to your staff on topics such as diabetes, dementia, incontinence, etc.
