Consultant Pharmacists

SeniorMed offers your residents the opportunity to have a Consultant Pharmacist conduct medication reviews. This may help seniors avoid medication-related complications. It is especially helpful for residents taking multiple medications prescribed by various doctors who may not communicate with each other.

For your residents, a Consultant Pharmacist:

  • Ensures a resident’s medication therapy is appropriate, effective, safe and used correctly
  • Identifies, resolves, and prevents medication-related problems such as drug interactions, drug-food interactions, dosing or therapeutic duplications that may interfere with the goals of therapy
  • Provides medication recommendations related to specific diseases or conditions that effect the senior population
  • Identifies medications that may be unnecessary and/or cause adverse side effects
  • Ensures adherence to the Beers’ Criteria for safe utilization of medications specific to the senior population
  • Performs risk assessments to identify seniors at risk for behavioral disturbances, falls, diabetes and cognitive impairment
  • Communicates with prescribers to suggest therapies and medications that would be safer and/or less costly to patients
  • Advocates wellness through health screenings and immunizations

For your community, a Consultant Pharmacist:

  • Observes adherence to medication pass and medication ordering procedures
  • Assists with regulatory compliance related to surveyor’s issues
  • Provides your community with medication information specific to the senior population
  • Reviews and provides recommendations for your community’s medication room, medication storage and medication destruction
  • Provides educational programs to your staff on an array of health-related topics such as diabetes, dementia, incontinence, etc.
  • Helps to prevent a resident’s transition to a higher level of care facility (for example,. nursing home) by promoting wellness, independence, and medication management
